A new report launched today (15 June) at Confed17, looks to better understand how to build trust across systems to promote more effective collaborative working | NHS Employers
Trustworthy Collaboration, written for NHS Employers by Prof Veronica Hope-Hailey of the University of Bath, explores the challenges of building trust across health systems, in the context of the 50 vanguard sites tasked with delivering transformational change through new care models.
The report looks into the challenges facing the leaders of the partner organisations within the vanguards, who are required to collaborate to deliver transformational change. Trust is essential to collaboration, the report states, but achieving this is easier said than done.
For the vanguards, trust was an important ingredient for ensuring success. It was vital for senior leaders to swiftly establish trust with other leaders across multiple and complex systems.
The report explores the meaning of trust in organisations and offers six key recommendations for how to successfully achieve it.
I have recently returned from an exciting, whirlwind tour of another set of new care models sites. This was my fourth such tour. My goal for these visits, which I make as an International Visiting Fellow for The King’s Fund, is to attempt to understand what the vanguard organisations are trying to do, how well they are faring, and how they might progress even better and faster. On this trip, the additional question was: ‘How can these lessons and models be spread more widely across the NHS?’

The vanguards are partnerships of NHS, local government, voluntary, community and other organisations that are implementing plans to improve the healthcare people receive, prevent ill health and save funds.
Considerable progress has been made since the vanguards were launched in 2015 and there is emerging evidence that they are making significant improvements at a local level. This includes reducing pressure on busy GP and A&E services.
In addition to the funding, the vanguards will continue to receive support from NHS England and other national bodies to implement their plans, including how they harness new technology including apps and shared computer systems. They are also receiving help to develop their workforce so that it is organised around patients and their local populations.
Here, in one of the most rural parts of the National Health Service, our clinicians are not just blessed with an excellent bedside manner. Increasingly, they are also becoming skilled in having a “webside manner” – as we harness the full potential of telemedicine to test out and develop a new model of care for the NHS.
We have adopted the same technology that NASA uses to ensure the health and well-being of their astronauts, like Major Tim Peake, as they orbit the Earth. But our approach is generating significant benefits not in space but much closer to home, as part of a project that is unprecedented in its size and scope.

The 23 vanguard sites chosen to develop the multispecialty community provider (MCP) and primary and acute care system (PACS) new care models have been working to pool budgets and integrate services more closely. Some are continuing to use informal partnerships, but others are opting for more formal governance arrangements. Commissioners are grappling with how to contract for the new systems, while providers are exploring how to work together within emerging partnerships, how to allocate funding, and how to share risk and rewards.
Many of the vanguard sites would like to bring together the budgets for core primary care services and other local services, but it seems unlikely that GPs will give up their core General Medical Services or Personal Medical Services contracts in the immediate future.

Many commissioners plan to contract with a single provider or entity to hold the budget and oversee or deliver a broad range of services, although most are still deciding which organisation or partnership should do this.
Commissioning and developing new care models involves risks as well as opportunities, underlining the importance of how these models are governed, their organisational form and how risks are shared.
Building and strengthening collaborative relationships is just as important as focusing on the technical elements of integrated care.
A new framework outlines how place-based partnerships can replicate the successful work of the 14 multispecialty community provider (MCP) vanguards, when establishing their own programmes.
Described as a milestone in the new care models programme and the implementation of the Forward View, the NHS England document defines what being an MCP means by taking features from the 14 vanguards to create a common framework. Commissioners and providers can follow the framework when establishing MCPs, and the vanguards themselves will adopt or adapt it for their own local communities.
The MCP vanguards will move specialist care out of hospitals and into the community, removing divides between primary, community, mental health, social care and acute services to provide efficient, joined-up and preventative care.
Highlighted in the framework is primary care home, the joint programme between NHS Confederation and the National Association of Primary Care (NAPC), which redesigns primary care around the needs of local communities with the same goal of providing integrated, joined-up and person-centred care.
